Reinstatement Request and Election of Benefit

HOW TO SUBMIT A REINSTATEMENT REQUEST OR ELECTION OF BENEFIT

If you are a "Cancelled Member" meaning that your membership at a Massage Envy Franchise was cancelled, terminated or not renewed between March 7, 2015, and June 30, 2016, and you have at least one unused monthly 50-minute membership massage resulting from your membership cancellation, termination, or nonrenewal, you may submit a Reinstatement Request to request reinstatement of your Unutilized Massages.

All Active Members of any Massage Envy Franchise as of June 30, 2016 who do not opt out of the proposed Settlement will automatically receive 60 days following membership conclusion to use or transfer all accrued unused massages under the terms of your membership agreement. Alternatively, if you are an Active Member who does not opt out of the proposed Settlement, you may instead complete an Election of Benefit to receive a 180-day period after membership ends to use 75% of any accrued unused massages as of the date your membership terminates, is cancelled or not renewed. If you elect this option, those massages cannot be transferred and must be used by you at your Home Clinic, pursuant to the Settlement Agreement's terms.

Both Cancelled Members and Active Members are Class Members.

You may complete and submit a Reinstatement Request or Election of Benefit Form electronically. Filing electronically is the easiest and quickest way to request reinstatement of your Unutilized Massages. Alternatively, if you would like a copy of the Reinstatement Request form to fax, email or mail to the Settlement Administrator, you can call 1 (888) 372-6327 to obtain it.


Stop! Before you submit your Reinstatement Request or Election of Benefit, you will need the following:

  1. Your Unique ID Code is located on the notice email or postcard you recently received. If you did not receive a notice but believe you are a Class Member or have misplaced your Unique ID, contact the Settlement Administrator at 1-888-372-6237. If you are a Class Member, the Settlement Administrator can provide you with your Unique ID Code.
  2. If you are a Cancelled Member and have moved more than 25 miles from your Home Clinic and also want to make a New Home Clinic Designation, you will also need:
    1. Third-party evidence of your current address (mortgage bill, phone bill, cable bill, driver’s license, etc.) in electronic format (PDF, JPEG, TIFF) that shows you moved more than 25 miles from your old Home Clinic.

Please note that if you were an Active Member meaning you were a a member of any Massage Envy Franchise as of June 30, 2016, you do not HAVE to take action at this time to receive benefits under the proposed Settlement if it is finally approved. All Active Members will automatically receive additional time in which to redeem any unused massages as of the date your membership terminates, is cancelled or not renewed.
